CHICAGO TWENTY20 CRICKET LEAGUE 2021 WAIVER FORM YEAR: 2021
CHICAGO TWENTY20 CRICKET LEAGUE PARTICIPATION AGREEMENT LIABILITY RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE
This is a legally-binding Release Waiver, Discharge and Covenant Not to Sue (collectively, “Release”), made voluntarily by me to the CHICAGO TWENTY20 CRICKET LEAGUE.
I, desire to participate in Chicago Twenty20 Cricket League, year 2021 as part of the team. This Assumption of Risk, Waiver, and Release from Liability covers the entirety of my participation in the Cricket sport including travel to any locations off of the Chicago Twenty20 Cricket League in order to participate in activities associated with the Chicago Twenty20 Cricket League. It is team’s Captain and Vice Captain’s responsibility to communicate the waiver requirement with all the players listed in their team.
1. Assumption of Risk: I am participating in the Chicago Twenty20 Cricket League at my own free will. I understand that my decision to participate in the Chicago Twenty20 Cricket League voluntary. I assume full responsibility for all risks that may arise out of or result from my participation in the Chicago Twenty20 Cricket League.
2. Waiver and Release: I, the Participant, release and forever discharge and hold harmless Chicago Twenty20 Cricket League and it successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from participation in the Chicago Twenty20 Cricket League activities. I understand and acknowledge that this Release dischargers Chicago Twenty20 Cricket League from any liability or claim that I may have against Chicago Twenty20 Cricket League with respect to bodily injury, personal injury, illness, death or property damage that may result from the participation in the Chicago Twenty20 Cricket League or occurring during my participation.
3. Insurance: I understand that I am solely responsible for any medical, health or personal injury costs relating to my participation in the Chicago Twenty20 Cricket League. Further I understand that the Chicago Twenty20 Cricket League does not assume any responsibility for or obligation to provide me with financial or other assistance including but not limited to medical, health or disability benefits or insurance or any nature in the event of my injury, illness, death or property damage. I expressly waive any such claim for compensation or liability on the part of Chicago Twenty20 Cricket League in the event of such injury or medical expenses incurred by me.
4. Medical Treatment: I hereby Release and forever discharge the Chicago Twenty20 Cricket League from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my participation in sports activities with the Chicago Twenty20 Cricket League.
5. Personal Injury: I hereby agree that I will not hold any participating individual’s and committee member’s responsible for any type of injury caused to me before, during or after the CHICAGO TWENTY20 CRICKET League games.
6. Payment for Damages: I agree to pay for any and all damages to any property or Release Party caused by me negligently, willfully or otherwise.
I, have read and fully understand this entire Liability Release; Waiver, Discharge and Covenant Not to Sue and understand that it relate to surrendering and releasing legal rights. I fully understand it and I agree to be legally bound by it.
